Australian Federal Police Association Life Insurance Cover
NON-EXCLUSION BASED LIFE ASSURANCE COVER TO THE VALUE OF $100,000 TO ALL FINANCIAL MEMBERS

Members and spouses may select voluntary cover of up to 5 units of insurance in addition to their basic level of cover (members only).
All voluntary and spouse cover is subject to satisfactory health evidence. Initially this will be in the form of a Personal Statement. We reserve the right to call for further medical evidence depending on the answers in the Personal Statement.
Voluntary/spouse cover begins on the date we notify to the policyowner.
The fortnightly premium for each unit of additional or spouse cover is unchanged at $2.00 per unit per member.

LIFE ASSURANCE - CONTINUATION OPTION
Hannover Life Re of Australasia Ltd is a professional reinsurance company and does not issue individual policies. However, we have an arrangement with Lumley Life Insurance Limited to provide continuation of cover under our group life policies.
Within 30 days of leaving service a member has the option to continue death cover under an individual policy with Lumley Life up to the age of 65 years.
This policy will be issued without health evidence, provided
(a) The member is under age 60 at the date of leaving service, and
(b) The member is not leaving service to join the armed forces of any country or does not intend joining the armed forces of any country, and
(c) A claim has not been submitted or one is not going to be submitted in respect of a member under the group policy.
Cover under the policy with Lumley Life is subject to,
(a) The member completing such forms including a satisfactory Aids Declaration as Lumley Life nominates and providing it with any personal information it requires, and
(b) The member paying the premium under the policy, and
(c) The amount of cover under the policy being no more than the final amount of accepted cover under the group policy or $750,000 whichever is the lesser, and
(d) Any conditions, restrictions or loadings that apply to the member under the group policy applying under the other policy, and
(e) Cover under the other policy continuing up to the member’s agreed cover ceasing age under the group policy or their 65th birthday whichever is the earlier.
Non-smoker rates may apply under the other policy where the member has not smoked tobacco or any other substance in the last 12 months.
Continuation Option Process.
Association must notify Hanover the following requirements.
A letter confirming the member is a financial member then detail name, address, date of birth and date the member is leaving the force.

Group Trauma Insurance
Definition
Pays a lump sum in the event of a diagnosis insurable condition, e.g. cancer, stroke, heart attack, major head injuries.
Pre existing conditions diagnosed prior to the 1st January 2006 will not be considered under this policy.
Purpose
If a person suffers a medical trauma, term life insurance won't help as the person is still alive. Suppose a person has a mild heart attack and is seriously ill but is able to return to work after two months. Will he or she be as productive as before or stand the stress associated with working as hard? What is needed is a cover like trauma insurance which pays out on the actual occurrence of the incident.
Trauma Insurance (also known as critical illness insurance) provides a cash lump sum in the event of contracting a specified disease or trauma.
The Group Trauma policy will pay benefits in the case of all of the following critical illnesses:

- Cancer
- Chronic Renal Failure
- Coronary Artery Bypass Surgery
- Critical Illness
- Heart Attack ( Myocardial Infarction )
- Major Organ Transplant Surgery
- Stroke
- Chronic Liver Disease

- Major Head Trauma
- Severe Burns
- Total Blindness
- Total Deafness
The function of trauma insurance is to:
- Help pay for specialist or international medical attention
- Help cover the cost of modifications to the home
- Help avoid financial stress in recuperation (debts, etc)
DEFINITIONS OF TRAUMA CONDITIONS
The following trauma conditions and their definitions will be covered:
(a) Cancer
Means the presence of one or more malignant tumours including leukaemia, lymphomas and Hodgkin's disease as characterised by the uncontrollable growth and spread of malignant cells and the invasion and destruction of the normal tissue.
Diagnosis must be by a Medical Practitioner approved by us.
The following tumours are excluded:
- Tumours showing the malignant changes of carcinoma in situ (including cervical dysplasia CIN-1, CIN-2, and CIN-3) or which are histologically described as pre-malignant *;
- Carcinoma in situ of the breast is covered if it results directly in the removal of the entire breast. The procedure must be performed specifically to arrest the spread of malignancy, and be considered the appropriate and necessary treatment.
- All skin cancers, unless there is evidence of metastases or the tumour is a Malignant melanoma of at least Clark level 3, or greater than 1.5mm maximum thickness as determined by histological examination using the Breslow method;
- Prostatic cancers which are histologically described as TNM Classification T1 or are of another equivalent or lesser classification, unless resulting in the surgical removal of the prostate;
- Papillary Micro-Carcinoma of the thyroid or bladder; and
- Chronic Lymphocytic Leukaemia less than RAI Stage 1.
(b) Chronic Renal Failure
Means end stage renal failure as diagnosed by a Medical Practitioner approved by us presenting as chronic irreversible failure of both kidneys to function, as a result of which regular renal dialysis is instituted.
(c) Coronary Artery Bypass Surgery
Means coronary artery bypass graft surgery considered medically necessary by a Medical Practitioner approved by us, performed in an open heart operation for coronary artery disease causing inadequate myocardial blood supply, but does not include laser therapy angioplasty or any other intra-arterial procedure.
(d) Heart Attack (Myocardial Infarction)
Means the death of a portion of the heart muscle as a result of inadequate blood supply to the relevant area. The basis of the diagnosis by a Medical Practitioner approved by us will be:
- Confirmatory new electrocardiogram (ECG) changes, and;
- A diagnostic rise and fall (other than as a result of cardiac or coronary intervention) in either Troponi I in excess of 2.0 ug/L or Troponin T in excess of 0.6 ug/L or cardiac enzyme CK-MB.
If the above criteria are not met, we will pay a claim based on satisfactory evidence that the Life Insured had unequivocally been diagnosed by a Medical Practitioner approved by us as having suffered a myocardial infarct resulting in:
- A permanent reduction in the Left Ventricular Ejection Fraction to less than 50%, measured 3 months or more after the event, or;
- New pathological Q waves.
(e) Major Organ Transplant Surgery
Means the human to human organ transplant from a donor to the Life Insured of one or more of the following organs:
- Heart
- Kidney
- Liver
- Lungs
- Pancreas
Or, the transportation of bone marrow.
The transplantation of any other organ, only part of an organ or any other tissue transplant are excluded from this definition. The operation must be considered medically necessary by an appropriate Medical Practitioner approved by us.
(f) Stroke
Means the first occurrence of a cerebrovascular event producing neurological deficit as diagnosed by a Medical Practitioner approved by us. This requires clear evidence on a CT, MRI or similar, appropriate scan or investigation that a stroke has occurred and of infarction of brain tissue, intracranial and/or subarachnoid haemorrhage, or embolisation from an extracranial source.
Transient ischaemic attacks, reversible neurological deficit, cerebral symptoms due to migraine, cerebral injury resulting from trauma or hypoxia and vascular disease affecting the eye, optic nerve or vestibular functions are excluded.
(g) Chronic Liver Disease
Means end stage liver failure together with permanent jaundice, ascites and encephalopathy.
(h) Chronic Lung Disease
Means end stage lung disease requiring permanent supplementary oxygen with FEV1 test results of consistently less than one litre.
(i) Major Head Trauma
Means neurological deficit resulting from trauma, causing at least 25% impairment of whole person function lasting more than three weeks from the date of the trauma, and likely to persist, as certified by a consultant neurologist.
(j) Severe Burns
Means full thickness burns to:
- At least 20% of the body surface area
- Both hands, requiring surgical debridement and/or grafting or
- The face, requiring surgical debridement and/or grafting.
(k) Total Blindness
Means the complete and irrecoverable loss of sight in both eyes as currently defined by the Royal Blind Society and confirmed by an ophthalmologist.
(l) Total Deafness
Means total, irreversible and irreparable loss of hearing both natural and assisted, in both ears as a result of disease, illness or injury.

BUSINESS DESCRIPTION:Australian Federal Police Employees
Operation of Cover:The Insured being Provided under this policy shall only apply whilst on a Journey as defined.
Scope of Cover: Wherever used in this
policy “Journey” shall mean “a Journey” undertaken by an Insured Person in the course of his employment with the Policyholder. Such cover shall commence from the time the Insured Person undertakes Direct Travel and cease upon arrival at either their normal residence or normal place of employment, whichever is their destination.
** Direct travel shall mean travel to and from the Insured Person’s normal residence or normal place of employment and shall include any minor deviations or interruptions which no way increase the risk of Accidental Bodily Injury that would have normally arisen had the Insured Person travelled direct
