1) I authorise the AFPA to arrange with my employer the transfer of my tax-deductible fortnightly subscription to the AFPA from my salary adjusted in accordance with the AFPA rules.
2) I authorise the AFPA to represent my employment interests in accordance with the Fair Work Act, the Fair Work (Registered Organisations) Act and the AFPA's rules and policies.
3) The AFPA Authority to Deduct Subscriptions from Salary and AFPA Industrial Agent Authority will remain in force until revoked by me in writing.
